The simplest pattern that works: each person takes one full week of on-call duty, then rotates to the next person.

Handoff timing matters. Do handoffs on Tuesday or Wednesday mornings, not Mondays or Fridays. Monday handoffs mean the new on-call person inherits weekend issues they have no context on. Friday handoffs mean the outgoing person mentally checks out before actually handing over. Mid-week gives both people a buffer.

Follow-the-sun (best for distributed teams)

If your 3-5 person team spans timezones, you can split coverage by geography instead of by week.

Business-hours-only (best for teams under 4)

If you have 3 or fewer people, 24/7 on-call will burn out your team fast. A 3-person rotation means each person is on-call every third week, including nights and weekends. That's not sustainable for more than a few months.

Instead, set on-call hours to 8am-10pm local time, Monday through Friday. For nights and weekends, configure your monitoring to:

  1. Auto-acknowledge non-critical alerts and batch them for the next business day.
  2. Only page for true outages (full service down, not degraded performance).
  3. Route critical after-hours alerts to a rotating "weekend backup" who gets compensated for the disruption.

This isn't a compromise. For many B2B SaaS products, most users are active during business hours anyway. Match your on-call coverage to when your customers actually need you.

Escalation policies: keep it to 2 levels

I've seen 5-person startups configure 4-tier escalation policies because that's what the PagerDuty documentation showed. Four tiers for five people means almost everyone gets paged for every incident. That's not escalation, that's a group chat.

The 2-level template for small teams

Level 1: Primary on-call (immediate)

Add a second tier when:

  • Your team hits 8-10 people.

Timezone handling for small distributed teams

If your team is spread across 2-3 timezones within the same region (say, US East and US West), follow-the-sun is overkill. Instead:

  • Set all schedule times in UTC. This removes ambiguity about when shifts start and end.
  • Aim for overlap windows of 2-4 hours where both the outgoing and incoming on-call person are awake. Handoffs happen during this window.
  • Respect local time for paging. An alert at 11pm UTC is 6pm Eastern but 4pm Pacific. That matters.
